In 2021, the British Maritime Investigation Bureau (UK MAIB) received more than 200 reports on incidents related to the soft ladder of air -citizers. Among them, 87%of the soft ladder was not installed correctly, and the rest was found to have major defects. MAIB inspector Bill Evans said that the pilots play a key role in the safe operation of the port, and they almost guide each ship to enter and exit the port. The number of pilot ladder should be in a good state and regular inspections. In many cases of the report, the side rope of the navigation ladder has a wear or damage, especially at the lower end (see Figure 1 and Figure 2).

After the report, it is determined that the charging ladder failure is caused by the poor condition of the side rope. In this case, conventional visual inspections will almost be discovered. MAIB reminds that if the rope on the cited ladder is worn or aging at any degree, it should not be used again.
